Join the Caredara Care-Ready Network™
Qualified Personal Support Workers, Registered Practical Nurses and Registered Nurses can join a network designed for rapid, short-term in-home stabilization in Ontario communities.
Activation-focused work
Caredara’s model is organized around defined care gaps such as discharge transitions, unexpected cancellation and temporary caregiver disruption. Each opportunity should match the practitioner’s qualifications, availability and scope.
Professional boundaries
Practitioners remain responsible for working within applicable professional standards and the scope of the accepted assignment. Medical emergencies are not stabilization requests and require the appropriate emergency response.
Supporting a continuity transition
The practitioner’s role is to help stabilize the short-term situation and support a clear handoff when the existing provider, community service or ongoing plan takes over.
