Caredara™ is a care activation platform focused on rapid, short-term in-home stabilization. It helps activate qualified PSWs, RPNs and RNs when an immediate care gap occurs, then supports a transition to the appropriate ongoing continuity provider.

The activation gap

Many care breakdowns happen before ongoing services can be arranged or restored. A patient may be medically ready to leave hospital while the home setting is not yet operationally ready, or an established caregiver may become unavailable without warning.

Caredara is designed for this short activation gap between an immediate non-emergency care need and reliable continuity of care.

From disruption to continuity

The pathway is simple: identify the care disruption, activate an appropriately qualified practitioner, stabilize the short-term situation and complete a warm handoff to the existing provider, a preferred continuity provider, a community service or the family care plan.

Caredara is not an ongoing home-care agency and does not replace hospitals, publicly funded services or long-term providers.

Who Caredara supports

Families can seek short-term help after discharge or an unexpected disruption. Hospitals, home-care organizations, employers, insurers and community partners can explore activation pathways and pilot programs. Qualified practitioners can join the Care-Ready Network™.
