  • Select the type of care needed (PSW/RPN/RN).
  • Book instantly or schedule in advance.
  • Practitioner arrives, checks in, and completes the visit.
  • Rate the practitioner at the end.

Families can also rebook the same practitioner for continuity.

How to Refer a Patient or Client to Caredara™

While formal tracking will be added later, the referral process for now is simple and manual.

Provide Client/Family With Info

Give the patient or family our referral card or email template so they can book care themselves through the app.

Submit a Direct Referral (Manual)

Send the referral details to:

Our team will follow up with the client or family within 1 hour.

Warm Handoff at Discharge

A care coordinator or discharge planner walks the family through downloading the app at bedside.
